Unsafe hospital care is causing an unacceptably high amount of harm to Australian hospital patients. This problem has now been recognised for more than 20 years, but sporadic interventions and the best of intentions have had frustratingly little success in closing the gap between best practice and current outcomes. In this presentation, the presenters will present data about the prevalence of safety issues in Australian hospitals, primarily drawing on routinely collected data.
Quality improvement efforts of Australian clinicians, hospitals, health departments and professional bodies have been stymied by a lack of actionable information about patient safety. The presenters argue that the lid needs to be lifted on patient safety in order to address this urgent challenge.
